2020-2021 School Calendar - This list of key dates is tentative and is subject to change. Always consult the calendar on the school website for the most complete and accurate information.
During the summer, families will receive emails directing them to this page on the website for updated back-to-school information and forms that will need to be completed before the beginning of school. All forms need to be submitted to school through the online Magnus Health portal. (See below for more information.) If you are having difficulty accessing your account, please email Cayce French.

Update Your BackPack Account (All Families)
We need your help to be sure CDS has the most up to date, accurate and complete contact and emergency information for your family.
UPDATE FAMILY DATA ON YOUR “MY BACKPACK” ACCOUNT
If you do not know your User Name and Password, click on "I Forgot My User Name/Password" and the information will be emailed to you.
Please be sure to update or add the following information on your My BackPack profile:
- Indicate any changes to your family contacts including address, cell phone numbers, email addresses, etc.
- Additional Contacts
- Emergency Contacts: family/friends who should be called if school is unable to reach parents. (Be sure to check the box under "Additional Contacts" to designate each emergency contact person.)
- Drivers: persons authorized to pickup your child from school. We must have your permission in order to release your child(ren) to another adult. Please list the names of your carpool and any other adults who may pick your children up from school for any reason. (Indicate "Driver" in the "Relation" field for each contact.)

Parents have the ability to control how much, if any, of their contact information is shared through this online directory. To block any of your personal/family data from being visible on this online directory, click the small DNP box next to each item. All student Magnus Health accounts are now active.

Carrollwood Day School partners with Magnus Health to collect and store your student’s health and medical information as well as required school forms for the upcoming school year. Magnus Health has released a new mobile app. Parents should use the PHR Mobile App to complete all requirements, fill in student health data, upload forms, and receive app notifications from the school regarding their students all on their phone! See the attached flyer for more information and instructions on how to set up the app on your phone.

New for 2020-2021:
- This year CDS has added a “COVID-19 Questionnaire” as a new requirement. This brief form will need to be filled out electronically on Magnus Health for each student. This is a self-reporting form and does not require a physician’s signature.
- Each student must also have a "Temperature and Health Screening Consent Form" completed. This form will need to be filled out electronically on Magnus Health and does not require a physician's signature.
- Magnus Health no longer accepts forms via fax or email. All documents must be uploaded through the new mobile app.
2020-2021 Required Forms and Instructions
Be Prepared!:
- Check requirements on Magnus before any doctor appointments to download and print all forms requiring a physician’s signature. These include forms for students who will need to take medication during the school day or have a special medical condition as well as the FHSAA Preparticipation Physical Examination form for athletes in grades 6 - 12. Your physician will provide the school entry physical form (FL DH 3040) and immunization form (FL DH 680). [Florida Immunization Requirements]
How to submit required forms:
- Use your "My BackPack" user name and password to log in to Magnus Health.
- You will see your child(ren) listed on your Front Desk screen. Click on the "Complete Now" button for each child.
- You will advance to a screen with initial "yes" and "no" questions about your child's age and activities.
- Once these conditional questions have been answered, the system will generate a customized "To Do" list with all the forms and other required information (like birth certificates and immunization forms) that need to be completed/submitted for your child.
- You must complete all questions on the forms; for example: if you are asked to list all allergies your child has and your child does not have any allergies, you will need to write "None." The system will not advance to the next page if any questions are left blank.
- You do not need to complete/submit everything in one sitting. Magnus Health will track your progress and will send email reminders to you weekly until all required forms and information is complete.
All forms and information must be completed/uploaded to Magnus Health by July 31, 2020.
CDS Dress Code
Carrollwood Day School is a uniform/dress code school for students in kindergarten through high school. The responsibility for assuring students are in the proper dress ultimately lies with the parents and the individual student. The school expects all students to be dressed appropriately on campus. Students may not be allowed to attend classes unless in appropriate uniform. Students who are not in full uniform for the first time will be loaned appropriate dress, if available. The second offense will result in the student’s parents being called.
Risse Brothers is the school uniform provider for Carrollwood Day School. Risse Brothers has a local retail outlet located at 4228 N. Armenia Avenue, Tampa, FL 33607 (phone 813. 282.8338). Risse Brothers' local store has a special department for CDS uniforms where selections can be tried on and purchased. Uniforms can also be purchased online at www.rissebrothers.com.
Risse Brothers is open for in-store and online shopping all summer. Shipping and curbside pick-up are available for online orders. Click here to read about the safety measures Risse Brothers has in place.
The approved uniform apparel varies slightly for each school level. More detailed information is included under the section of the student handbook for each school division.

General Uniform Reminder
Remember that part of the purpose behind wearing uniforms is to create a conservative school environment. In a very few cases, the skort/short that fits waist and hip may be too short or too long. In these cases, altering may be necessary. Please allow for growth during the year. If the uniform is not the appropriate length, the student will not be allowed to wear the uniform to school.
Hair
All students’ hair should be kept neat and clean. A boy’s hair must not cover the eyebrows or touch the collar. Those who fashion hairstyles that are unruly will be required to have their hair cut to an appropriate length or tied back. Words and designs may not be cut into the hairstyle.
Hats & Hoods
CDS hooded sweatshirts may be worn in school; however, hats and sweatshirt hoods are not allowed to be worn in the school buildings. Hats and sweatshirt hoods may be worn outdoors during lunch, P.E., or recess. Students who consistently challenge this rule will not be allowed to bring hats or hooded sweatshirts to school or their hat or hooded sweatshirt will be held in the office or by the Assistant Division Head.
Jewelry
Any jewelry must be appropriate for outdoor play. CDS will not be responsible for the loss of any jewelry brought to school by students. Students may only wear up to two pairs of earrings. No other type of visible body piercing or tattoos are permitted.
Additional Information on Uniforms
- Skorts/jumpers/shorts are to be worn within 4” of the back of the knee.
- Shorts must not be too tight.
- Be sure that buttons on shorts, pants, and skorts are secure and that zippers work.
- Students may not wear their P.E. shirt under their uniform shirt. § Closed-toe shoes appropriate for school activities must be worn each day.
- Students will be advised when they may wear or change into a team uniform.
- Pants and shorts may not be housed (pulled below the waist).
- High heels/platform shoes, sneakers with wheels (heelies), sandals, boots, and “crocs” are not appropriate school attire.
- Bottom garments are to be worn loose enough to tuck in shirts without wrinkling and to allow for easy body movement including comfortable sitting.
- Shirts worn under the uniform top must not be visible.
- Wearing of bandanas is not permitted.
- Only the oval “Patriot Pride” eagle logo is allowed on PE apparel and sweatshirts.
- Any apparel that is inappropriate or distracts from the intent of the uniform is not permitted.
Non-Uniform Dress Code
Students will be notified when there will be non-uniform days. Half-days are not automatic non-uniform days.
- Students may wear solid T-shirts or T-shirts with college or sports team logos. Students may not display slogans, drug related pictures or music promotions, advertisements, or offensive pictures.
- Cut-off shirts, midriff tops or tank tops are not appropriate school attire.
- Closed shoes must be worn.
- Shorts and skirts cannot be more than 4 inches above the back of the knee.
- Jeans, slacks, or long shorts are acceptable. Ripped or torn jeans are not acceptable.
- Army fatigues, camouflage clothing or steel-toed shoes are not appropriate school attire.
- Pants and shorts may not be housed (pulled below the waist).
- No extreme clothing may be worn.
- We recommend that names be written on the inside of the neckband or waistband.
Dress Code Violations
Any student who is out of uniform or who wears an inappropriate form of the uniform will be asked to wait in the office until proper clothing can be brought to school. Please be sure that your child comes to school in his/her uniform so that he/she does not miss class time waiting in the office. All classes missed because of inappropriate clothing are unexcused. Consequences will be assigned as appropriate.
Lower School
SUMMER SUCCESS @ CDS
As we head into summer vacation, the elementary academic support team has prepared a resource packet with optional learning activities for your child to do over the summer. The full packet of flyers will be mailed to your home this summer. These flyers are designed to encourage your child to sharpen their skills in a variety of areas: Reading, S.T.E.M, and Social-Emotional Learning. Some of the platforms we have available for students this summer, such as IXL and Lexia, are individualized and adaptive. (The packet mailed to your home will include logins and passwords for online resources available to CDS students.) The Distance Learning Plan is also filled with incredible resources.

Grades 9 - 11 Language and Literature Summer Assignments
Students are required to read two books over the summer. One book will be assigned by their grade level and the second book will be a "choice" book. Students have the opportunity to indicate three books that they would like to read for their "choice" book from the list published by the Language and Literature Department. "Choice" books will be assigned on a first come, first served basis before the end of school. If a student's first choice is not available, they will be assigned their second or third choice. If students make no choice, the faculty will assign a book for them to read.

Forms as Needed (Athletics, After Care & Transportation)
Athletics
ATHLETICS ARE STARTING FOR MS & US ATHLETES!
Students in grades 6 - 12 who will be participating in athletics including preseason conditioning and practice are required to submit new forms every year. The athletic forms requirements can be found under "Athletics" on the Carrollwood Day School website; forms can also be picked up in the Hayes Family Gymnasium Athletic Office.
*** ALL PLAYERS MUST FILL OUT ALL THE REQUIRED ATHLETIC FORMS AND UNLESS OTHERWISE NOTED, ALL FORMS NEED TO BE UPLOADED TO MAGNUS HEALTH. THIS INCLUDES DOCTOR PHYSICALS AND THE VIDEOS. NO STUDENT ATHLETE WILL BE ABLE TO PARTICIPATE WITHOUT THESE FORMS TURNED INTO MAGNUS ***
All students who will be participating in conditioning and athletic practices during the summer must also turn in a 2020 Summer Consent, Authorization, and Release (CAR) Form, Temperature Check Consent Form, and COVID-19 Questionnaire. Transportation
Carrollwood Day School offers transportation to and from the Bearss Avenue Campus on a fleet of school owned buses. This service is maintained for the convenience of CDS families. CDS has established bus stops at various locations on each of our bus routes. Compliant with State guidelines, CDS may not pick up students at their residence. In addition to the route bus stops, there is a shuttle between the ECC and Bearss Avenue Campus to assist families with children enrolled at both campuses.
Due to this unprecedented time with the potential spread of COVID-19, CDS has reviewed our policies and procedures and adapted them to ensure health and safety for all. The following protocols have been put in place to keep our school buses safe for our students and drivers:
- Parents must check student’s temperature prior to boarding the bus
- Children's temperatures will be checked at school before they can enter their classroom; if their temperature is 100.4 or higher parents will be contacted to come pick up their child.
- Students will be required to wear approved face coverings on the bus
- Sanitizer will be available on the bus (for students in 1st grade and older)
- Social distancing will be strictly followed on the bus with one child per bench; occupancy will be limited to 20 students
- Weather permitting, windows will be kept open

For families that do not use the CDS bus service, Carrollwood Day School encourages all parents to carpool whenever possible. In an effort to "go green," the CDS Admissions team is glad to help parents to find carpool partners.
After Care
The opening of Before School and After School Child Care will be delayed until at least after Labor Day.
CDS uses KidCheck® to provide an online check in/check out system for students that participate in the extended day program at both the Early Childhood Campus (ECC) and the Bearss Avenue campus (grades 1 - 5). KidCheck® improves the safety and tracking of our children and will also enhance our communication, security and our administration of our extended day programs. Each family that intends to utilize the before care (ECC only) or after care program (both campuses) must create an account prior to the first day of attendance. If your child is already registered in KidCheck, it is not necessary to create a new account; your account is still active.
